Dental implants effectively restore lost teeth, but peri-implantitis, accompanied by bone resorption, reduces their success rate (according to the literature, the incidence is 10-20%). Bone substitutes, such as β-tricalcium phosphate (β-TCP) and bone collagen, are most often used to restore lost bone tissue in peri-implantitis. The aim of this study was to comparatively analyze the dynamics of bone density in the apical, thread, and cervical zones of implants in patients with peri-implantitis after defect replacement with these materials. The study included four groups (intact, a control group with peri-implantitis without replacement, and two experimental groups with β-TCP and collagen). Density was assessed by computed tomography in Hounsfield units at baseline, 3, 6, and 12 months using ANOVA and post-hoc tests (p<0.05). A biphasic trend was observed in the experimental groups: a decrease in density by 3 months followed by an increase by 12 months, especially in the threaded zone (up to 237.5±2.43 HU in the β-TCP group and 236.4±3.89 HU with collagen). The apical zone retained high density, while the cervical zone had minimal density. The materials demonstrated comparable efficacy in osteoregeneration, outperforming the control group. Zonal analysis confirmed the anatomical features and statistical significance of the differences. Conclusion: β-TCP and bone collagen are effective in bone tissue restoration in peri-implantitis, with an emphasis on the threaded zone. Zonal analysis using cone-beam computed tomography (CBCT) is recommended for assessing osseointegration.
