Rehabilitation of patients with extreme alveolar ridge atrophy and a history of implant failures represents one of the most challenging problems in oral and maxillofacial surgery, when traditional methods of dental implantation and bone grafting are not effective. This article presents an innovative combined treatment protocol illustrated by the case of a 69-years-old female patient with extreme atrophy of the upper and lower jaws and a history of unsuccessful previous implantations. The uniqueness of the method lies in the comprehensive application of digital planning, transnasal and zygomatic implantation in the upper jaw, as well as the immediate placement of a custom titanium subperiosteal implant, fabricated using digital technologies, in the lower jaw. The obtained results demonstrated achieving primary stability, absence of complications, and the possibility of fixed prosthetics within the shortest timelines. The presented protocol shows high effectivity and predictability in rehabilitating patients in clinically challenging situations with no alternatives, opening new perspectives in oral and maxillofacial surgery.
