Relevance. An increase in the concentration of pro-inflammatory cytokines during the persistence of periodontopathogenic microflora in the area of dental implants affects the process of bone tissue resorption. Material and methods. We studied 75 patients aged from 27 to 72 years with integrated dental implants loaded with orthopedic structures for 5 years. Patients are divided into 3 groups depending on the severity of bone resorption in the area of dental implants: less than 1/3, 1/2, more than 2/3 of the implant length, respectively. The patients underwent a questionnaire, CBCT, microbiological examination, and the concentrations of IL-6 and TNF-α, EN-1 were assessed. Results. The concentration of the cytokine IL-6 corresponding to the degree of bone tissue resorption in the implant area was established: the greatest increase in the indicator was observed in group 3 (2.3±0.4 pg/ml) with the highest degree of resorption; in group 2 the indicator had a borderline value (1.3±0.5 pg/ml), in group 1 the indicator was within the reference values (0.34±0.31 pg/ml) (p<0.001). The level of TNF-α in all groups did not exceed the reference value, but in group 3 it was higher (2.3±0.2 pg/ml) than in group 2 (1.7±0.5 pg/ml) and group 1 (0.5±0.1 pg/ml) (p<0.001). The highest occurrence of microorganisms P. intermedia (85.7%), T. denticola (64.3%) was recorded in group 3 compared to groups 1 and 2 (p<0.001, p=0.010). Conclusion. A relationship was established between the clinical and radiological degree of bone resorption in the implant area with the concentration of cytokines IL-6, TNF-α and the occurrence of P. intermedia and T. denticola. Analysis of the cytokine profile may have prognostic significance when planning implantation due to the sensitivity of IL-6 and TNF-α to the persistence of P. intermedia and P. gingivalis.
