The aim of the study was to evaluate the clinical stability of a bar-retained overdenture supported by four mandibular dental implants at different screw tightening torque values. The clinical study included 15 patients aged 55-60 years with complete mandibular edentulism and Keller class I-II alveolar ridge atrophy. Four dental implants were placed in regions 33, 43, 36, and 46 followed by fabrication of bar structures using selective laser sintering or CAD/CAM milling. Screw fixation was performed with torque values of 15, 20, and 25 N·cm, as well as with a combined protocol using 25 N·cm in distal regions and 20 N·cm in the anterior region in patients with increased distal cantilever length. Prosthetic stability, peri-implant soft tissue condition, and screw loosening frequency were evaluated at 6 and 12 months. Torque values of 20 and 25 N·cm provided high clinical stability of the bar system, whereas 15 N·cm was associated with a risk of distal screw loosening, particularly in cases with increased distal cantilever length. The optimal range of screw tightening torque for bar-retained mandibular overdentures was 20-25 N·cm.
