Relevance: There are many factors that affect the effect of anesthetics, and one of them is the anatomical structure of the jaws and the number of micro–nutritional holes in the bone structure. Undesirable reactions may occur due to violations of the technique of anesthesia, and may also be associated with the anesthetic itself in case of incorrect choice of its dosage. The aim of the study is to clinically evaluate the effectiveness of 2% and 4% articaine in tooth extraction surgery. Materials and methods: 70 patients with diagnoses K04.5 «Chronic apical periodontitis» and S02.5 «Tooth fracture» were treated at the Lazurit Dental clinic from 2024 to 2025. According to the indications, all patients underwent tooth extraction in the upper jaw — premolars or the first molar. During the study, CBCT took into account the quantitative factor of micronutrient holes in the bone structure of the patients' jaws, and compared the anesthetic effect of 2% and 4% articaine by measuring EDI with the IVN-01 Pulptest-Pro device. EDI indicators were recorded at 0 minutes, after 2, 4 and 8 minutes. Changes in pain sensitivity in patients before and after administration of anesthetic during tooth extraction were recorded according to the EDI data of the causal tooth and adjacent teeth. Results: the age of patients in group 1 was 28-56 years, in group 2 — 26-56 years. When assessing the dynamics of EDI changes within each group, the following was noted: — in group 1 (2% articaine), a sharp increase in the index was noted by the 2nd minute after administration, with its further increase by the 4th minute and a gradual gradual decrease by the 8th minute; in group 2 (4% articaine), as well as in the 1st group, there was a sharp increase in the index by the 2nd minute after administration of the anesthetic and its gradual more gradual increase by the 8th minute after administration. All patients noted the absence of pain at all stages of tooth extraction surgery. The postoperative period was uneventful, and no complications were reported. Conclusions: correlations between the number of feeding holes in patients of different age groups and EDI values at 4 minutes suggest that there is a possible dependence of the number of feeding holes on the age of the patients. Taking into account the results obtained, indicating adequate anesthesia of the surgical area up to and including the 8th minute, 2% articaine demonstrated no less effectiveness than 4% articaine, therefore it can be recommended for use with short-term tooth extraction.
