Влияние комплекса лечебно-профилактических мероприятий на стоматологический статус детей, оказавшихся в трудной жизненной ситуации “социальных сирот”

Аннотация
Целью данного исследования явилась оценка эффективности действия комплекса лечебно-профилактических мероприятий на стоматологический статус и гигиеническое состояние полости рта у детей, оказавшихся в трудной жизненной ситуации и проживающих в социальных учреждениях.
В ходе исследования было проведено обследование 646 детей, оказавшихся в трудной жизненной ситуации и воспитывающихся в стационарных социальных учреждениях Республики Башкортостан, в возрасте от 6 до 16 лет. Исследование проведено в два этапа. На первом изучена распространенность и интенсивность кариеса (индекс КПУ), состояние тканей пародонта (индекс РМА) и гигиенический статус полости рта (индекс OHI-S). На втором этапе исследована эффективность комплекса лечебно-профилактических мероприятий на индексы КПУ, РМА и OHI-S в ключевых возрастных группах.
Показано, что у детей-сирот и детей, оставшихся без попечения родителей, воспитывающихся в социальных учреждениях, худшее стоматологическое здоровье, чем у детей в общей популяции страны. Распространенность кариеса повышается с возрастом от 84,3±2,67% у детей 6-7 лет до 89,3±2,18% у подростков 14-16 лет. Выявляется высокая интенсивность кариеса временных зубов у 6-7-летних воспитанников (индекс кп 5,95±0,224) и постоянных зубов у подростков 14-16 лет (индекс КПУ 4,84±0,232). В структуре индекса КПУ во всех возрастных группах превалирует компонент “К”, выявляются осложненные формы кариеса зубов (пульпит, периодонтит), характеризуя тяжесть лечения кариозного процесса. Медиана индекса гигиены полости рта соответствует неудовлетворительному во всех возрастных группах, а индекс РМА — гингивиту средней степени тяжести.
Внедрение разработанного комплекса лечебнопрактических мероприятий, включающего: стандартную терапию основных стоматологических заболеваний, обучение навыкам и повышение уровня культуры в области гигиены полости рта и профилактики стоматологической патологии, формирование устойчивой мотивации детей к сохранению и укреплению зубов, снижению распространенности и интенсивности кариеса зубов, заболеваний пародонта, а также использование ополаскивателя ротовой полости из водного настоя лекарственных трав, обладающего противовоспалительным, бактерицидным, ранозаживляющим, обезболивающим и кровоостанавливающим действием, оказало эффективное действие, улучшая гигиену полости рта, снижая интенсивность течения кариозного процесса зубов и воспаления тканей пародонта.

The purpose of this research was to evaluate the effectiveness of the complex of therapeutic and preventive measures on the dental status and hygienic condition of the cavity in children in a difficult life situation and living in social services.
An examination of 646 children raised in a difficult life situation in stationary social services of the Republic of Bashkortostan at the age of 6 to 16 years was conducted. The research was conducted in two stages. At the first stage, the prevalence and intensity of caries (DFE index), the condition of periodontal tissue (PMA index) and the hygienic status of the oral cavity (OHI-S index) were studied. At the second stage, the effectiveness of a complex of therapeutic and preventive measures on the DFE, PMA and OHI-S indices in various age groups was investigated.
It showed that orphans and children left without parental care, raised in social services, have worse dental health than children in the general population of the country. The prevalence of caries increases with age from 84.3± 2.67% in children aged 6-7 years to 89.3± 2.18% in adolescents aged 14-16 years. There is a high intensity of caries of temporary teeth in 6-7-year-old pupils (df index 5.95 ± 0.224) and permanent teeth in adolescents aged 14-16 years (DF index 4.84 ± 0.232). In the structure of the DFE index in all age groups, the “D” component prevails complicated forms of dental caries (pulpitis, periodontitis) are revealed, characterizing the severity of the treatment of the carious process. The median of the oral hygiene index corresponds to unsatisfactory in all age groups, and the PMA index corresponds to gingivitis of moderate severity.
The implementation of the developed complex of therapeutic and practical measures, including the treatment of major dental diseases, skills training and improving the level of hygiene culture in the field of oral hygiene and prevention of dental pathology, the formation of a stable motivation of children to preserve and strengthen teeth, reduce the prevalence and intensity of dental caries, periodontal diseases, as well as the use of an oral rinse from an aqueous infusion medicinal herbs with anti-inflammatory, bactericidal, wound healing, analgesic and hemostatic effect had an effective effect, improving oral hygiene, reducing the intensity of the course of the carious process of teeth and inflammation of periodontal tissues.
