Relevance. Choosing the optimal concentration of local anesthetic for tooth extraction requires evaluating not only the effectiveness of anesthesia, but also its effect on the body's regulatory systems, especially in patients with concomitant pathology. Goal. To compare the anesthetic efficacy of 2% and 4% articaine solutions with a vasoconstrictor during tooth extraction and to assess the stress of regulatory systems based on monitoring data of the cardiovascular and respiratory systems. Materials and methods. The study included 56 patients divided into two groups (n=28): in the first, 2% articaine was used, and in the second, 4% articaine was used. Blood pressure, pulse rate, and saturation (SPO₂) were monitored during the intervention stages. The depth of anesthesia was assessed using electro-dental diagnostics (EDI) and a visual analog scale (VAS). Results. An adequate level of anesthesia was achieved in all patients in the 4% group and 82.1% in the 2% group at the 2nd minute; by the 4th minute — in 100% in both groups. There were no statistically significant differences in the depth of anesthesia according to EDI and VAS data between the groups (p>0.05). Hemodynamic and saturation parameters in both groups remained within the physiological norm. The changes were transient, reflecting a typical stress response, with no clinically significant intergroup differences. No correlation was found between the quality of anesthesia and cardiorespiratory parameters. Conclusions. 2% and 4% articaine solutions demonstrate comparable clinical efficacy in tooth extraction and do not cause clinically significant stress on the cardiovascular and respiratory systems in patients with satisfactory somatic status. Given the comparable efficacy and evidence of lower cytotoxicity, a 2% solution of articaine can be considered as the preferred option for standard interventions to reduce the drug load.
