Согласованность объективной нуждаемости и субъективной потребности в лечении пациентов с дисфункцией височно-нижнечелюстного сустава

Целью исследования стала оценка динамики и согласованности числовых параметров, характеризующих объективную нуждаемость и субъективную потребность пациентов до и после лечения дисфункции височно-нижнечелюстного сустава. Материалы и методы: проведено клиническое обследование и анкетирование 55 пациентов c дисфункцией височно-нижнечелюстного сустава до и после проведения лечения. Объективная нуждаемость в лечении численно выражалась с помощью индекса Хелкимо. Субъективная потребность в лечении выражалась с помощью функциональной числовой рейтинговой шкалы (NRS). На основании полученных данных был проведен расчет коэффициента корреляции Спирмена. Теснота корреляций оценивалась по шкале Чеддока. Результаты: исходное значение медианы индекса Хелкимо составляло 17 баллов (ИКР 11-22), после завершения лечения — 7 баллов (ИКР 5-10,5). Значение медианы числовой рейтинговой шкалы (NRS) до лечения составляло 7 баллов (ИКР 6-9), после лечения — 2 балла (ИКР 1-2). Индекс Хелкимо и функциональная числовая рейтинговая шкала NRS лечения демонстрируют заметную прямую корреляционную связь до (0,56; p<0,05) и после (0,59; p<0,05) проведения лечения. Заключение: объективная нуждаемость и субъективная потребность в улучшении функции зубочелюстной системы достоверно снижаются после проведения лечения. Выявлена корреляционная зависимость между индексом Хелкимо, характеризующим объективную нуждаемость, и функциональной числовой рейтинговой шкалой NRS, отражающим субъективную потребность в лечении.

Objective: to assess dynamics and consistency of numerical parameters characterizing objective need and subjective demand in patients before and after treatment of temporomandibular joint disorder. Material and methods: clinical examination and questionnaire were conducted in 55 patients with temporomandibular joint disorders before and after treatment. The objective need for treatment was numerically expressed using the Helkimo index. The subjective demand for treatment was expressed using the functional numeric rating scale (NRS). Based on the data obtained the Spearman correlation coefficient was calculated. The correlations were interpreted using the Chaddock scale. Results: The initial median value of the Helkimo index was 17 points (IQR 11-22), after treatment — 7 points (IQR 5-10,5). The median value of the functional numeric rating scale (NRS) before treatment was 7 points (IQR 6-9), after treatment — 2 points (IQR 1-2). There was noticeable direct correlation between Helkimo index and the numeric rating scale before (0,56; p<0.05) and after (0,59 p<0,05) treatment. Conclusion: objective need and subjective demand significantly decrease after treatment. Correlation was noted between the Helkimo index characterized objective need and the functional numeric rating scale reflected the subjective demand for treatment.
