The long-term success of dental implantation depends not only on osseointegration but also on the condition of the surrounding soft tissues, which serve a protective function. Objective. To systematize and analyze current scientific literature data on the anatomical and histological features of the peri-implant mucosa in order to optimize surgical and prosthetic treatment approaches and prevent complications. Conclusions. The anatomical and histological architectonics and the biological barrier of the mucosa in the peri-implant zone are critical for the long-term success of dental implantation. Due to the absence of cementum, the parallel orientation of collagen fibers, and reduced vascularization, this region is more vulnerable to bacterial invasion and mechanical stress than the natural periodontium. The gingival biotype, width of keratinized tissue, and depth of the biological width serve as objective criteria for the stability of the marginal gingiva and bone. Disruption of this interface triggers mucositis and peri-implantitis, leading to subsequent bone resorption. Therefore, surgical and prosthetic protocols must necessarily incorporate soft tissue preservation or augmentation to ensure an adequate volume of keratinized gingiva.
